The Health form for my clients is just a basic personal information sheet so as the therapist, I, know your name, basic information, and who to call in the case of an emergency.

I also require you fill out a medical form, so I know of any medications or medical problems you may have. This allows me know what to do during your massage or if I can massage you at all. There may be certain medications or health issues that I wouldn’t be able to massage. For the most part it’s very rare that I wouldn’t be able to massage a client, because we all know that massage is wonderful and helps alleviate a lot of health issues
